- Inadequate oral intake related to swallowing difficulties as evidenced by involuntary weight loss of x kg in x months.
- Inadequate vitamin (vitamin D) and mineral (calcium) intake related to decreased ability to consume sufficient calcium and vitamin D, as evidenced by compromised bone mineral density.
